ADHD reflection and GP planner

Make space for the whole picture before you speak to your GP.

This private tool helps you put ADHD experiences alongside perimenopause changes and their impact on daily life. It creates a summary you control.

It does not diagnose ADHD, calculate a result, save your answers or replace a specialist assessment. [1]

Part one

Reflect on adult ADHD experiences.

These are reflection prompts based on common adult ADHD themes and the areas a specialist assessment considers. They are not a diagnostic checklist. Choose only the statements you want to take into a conversation. [2]

01Attention and organisation

These prompts use plain-language themes from NHS adult ADHD information. Select only what feels relevant to you.

02Restlessness and impulsivity

ADHD can include hyperactive or impulsive experiences, but not everyone experiences these in the same way.

03Looking back and across life

For an adult referral, NICE describes typical ADHD manifestations that began in childhood, persisted over time and affect more than one important setting. A specialist explores this in detail.

04Impact on daily life

A healthcare professional will consider the impact of symptoms, as well as other possible explanations and any coexisting conditions.

Part two

Add the perimenopause context.

This section lets you note the changes that may need to be considered alongside possible ADHD. It does not assume that either is the answer. [3]

Your conversation notes

A summary you can keep in your own words.

Select any prompts that feel useful. A preparation summary will appear here, without a score or label.

ADHD and perimenopause conversation notes
This is my personal preparation summary. It is not a diagnosis or a request for a particular outcome.

ADHD experiences I would like to discuss:
• I have not selected any ADHD reflection prompts yet.

Perimenopause and recent changes I would like to discuss:
• I have not selected any perimenopause planning prompts yet.

The impact or context I would like my GP to know:
• I will add this in my own words during the appointment.

I would value a conversation about what may be relevant, whether anything else should be considered and what support or next steps may be appropriate.
